Admission

Clienst receive a complete Nursing Assessment by an experienced RN at the time of admission.  This includes a review of medications, a home safety assessment, a fire plan, a fall risk assessment, a nutritional screening, disaster planning along with a physical review of the client’s functional status and assessment of physical systems.

Plan of Care

The nurse develops a Plan of Care with input from the patient, family, and caregivers.  The nurse includes cultural and religious influences in development of the plan of care. If a certified home health care aide is providing care, the Nurse directs the care.  The nurse will include the entire health care team in the plan of treatment. 

Supervision

The RN Home Care Supervisor visits at least every 60 days and as needed to supervise the provision of care according to the plan.  There is an experienced nurse on call 24 hours a day to answer questions that the family or client may have regarding their Plan of Care. The nurse provides education to the client and family as needed.
